Tooth Extraction On Obolon
What tooth extraction is
Tooth extraction is a minor surgical procedure in which a tooth or its roots are removed from the socket in the bone. It is done when a tooth can no longer be saved by treatment: it is broken down, cracked, very loose, or has become a source of inflammation that does not go away.
At VILIDA in Obolon, Kyiv, teeth are removed by the clinic’s oral surgeons. Before the procedure, the surgeon looks at an X-ray, either a panoramic one or a CT scan, both taken here at the clinic: it shows the shape and number of the roots and the condition of the bone and neighbouring teeth, so the plan is clear in advance. The area is numbed with local anaesthesia, and patients who would rather sleep through the procedure can choose deep sedation. Wisdom tooth extraction has its own page.

When a tooth is removed and when it is saved
The surgeon always checks first whether the tooth can be saved: your own root with a sound crown works better than any replacement. Extraction is suggested when treatment can no longer give a lasting result.
Extraction is needed if
- The tooth is broken down below the gum line, so neither a filling nor a crown can be fixed to it
- The root has cracked lengthwise: such a tooth cannot be restored
- The tooth is very loose because gum disease (periodontitis) has destroyed the bone around it
- There is inflammation at the root tip that could not be cured, and the root canals cannot be treated again
- The tooth gets in the way of straightening the bite and your orthodontist recommends removing it, or it is an extra (supernumerary) tooth
We try to save it first or postpone if
- The root canals can be treated, including retreatment under a microscope, and the inflammation at the root can be cured (apical periodontitis treatment)
- There is enough tooth left to restore it with an inlay or a crown
- You are pregnant or unwell, with an acute infection or a flare-up of a chronic illness: planned extraction is postponed
- You take blood-thinning medication: tell the surgeon in advance; only the doctor who prescribed it can change the dose
How much tooth extraction costs
The clinic’s price list divides tooth extraction into complexity categories. The surgeon sets the category after the examination and X-ray, based on the number and shape of the roots, how badly the tooth is broken down and whether an incision will be needed.
Tooth extraction prices
The doctor confirms the final cost after the examination.
The surgeon will tell you the cost of your particular case, including the X-ray and, if you choose it, sedation, at the examination before the extraction. X-ray prices are on the Price page, baby tooth removal is covered on the Baby Tooth Extraction page, and wisdom teeth on a separate page.
Simple and surgical extraction
A simple extraction is when the crown of the tooth is whole or partly preserved and the roots are straight: the surgeon separates the gum from the tooth, loosens it with an instrument and lifts it out of the socket. A surgical, or complex, extraction is needed when only the root is left, the crown breaks or the roots spread apart or are curved. The surgeon then makes a small incision in the gum, divides the tooth into separate roots, removes them one at a time and closes the wound with stitches.
Removing a root with almost no crown left above it is also a surgical extraction; the surgeon will set its category from the X-ray.
How a tooth is extracted
-
Diagnosis
The surgeon examines the tooth and the X-ray, assesses the roots and bone, and explains whether the tooth can be saved and, if not, how it will be removed and what it will cost.
-
Anaesthesia
After the injection the area goes numb within a few minutes; under sedation you sleep, and an anaesthetist monitors you.
-
Extraction
The tooth is lifted out whole or in separate roots, depending on what the X-ray showed.
-
Socket
The socket is cleaned and treated; if the tooth is to be replaced with an implant, it may be filled with bone graft material where indicated (bone grafting). A gauze pad goes on top, with stitches if needed.
-
Check-up
At the check-up the surgeon looks at how the socket is healing, removes any stitches and plans the tooth replacement with you.
What to do before an extraction is explained in our article on preparation.
How the socket heals, day by day
The socket heals gradually: first a blood clot closes it, then the gum, and the bone takes the longest to recover.
| When | What happens | What to do |
|---|---|---|
| First 2 hours | The anaesthetic is still working; a clot under the gauze pad closes the socket | Do not eat until the numbness wears off; keep the gauze pad in as long as the surgeon advised |
| First 24 hours | The socket may bleed a little and ache | Do not rinse, avoid straws and cigarettes; hold something cold against your cheek with breaks |
| Days 2–3 | Swelling and pain peak, then ease | Soft warm food, chew on the other side; painkillers as prescribed |
| Days 7–10 | New tissue covers the socket; stitches, if any, are removed | Brush carefully next to the socket; use only the rinses the surgeon recommended |
| 3–4 weeks | The gum over the socket heals | You can chew on that side as usual |
| Several months | New bone forms in the socket | Time for an implant if one was not placed on the day of extraction |
When to call the surgeon
Call the surgeon if the pain gets worse rather than better after a few days, the socket smells unpleasant, bleeding does not stop after 20–30 minutes of pressure with gauze, the swelling grows after the third day or your temperature rises above 38 °C. These can be signs of an inflamed socket (dry socket), which is treated at the clinic.
How to replace an extracted tooth
Apart from a wisdom tooth, the gap left by an extracted tooth should not stay empty: the teeth gradually shift, and bone without load becomes thinner.

- Implant. It replaces the root and does not involve the neighbouring teeth. If the bone around the socket is healthy and there is enough of it, the implant can be placed on the day of extraction; this is called immediate implant placement. Otherwise it is placed after healing, sometimes together with bone grafting. Implant systems and prices are in the Dental Implants section.
- Bridge. A crown in place of the tooth is held by the neighbouring teeth, which are reshaped for this (dental bridges).
- Removable denture. An option when several teeth in a row are missing and implants are not possible (clasp dentures).
The surgeon and prosthodontist will discuss what suits you before the extraction, so that you can plan right away how to preserve the bone for a future implant.
Tooth extraction under sedation
If fear of the dentist keeps you from treatment or several teeth need to come out, the extraction can be done under deep sedation: you sleep, the medicine is given into a vein, local anaesthesia also works, and an anaesthetist monitors you. Under sedation, several procedures can be combined in one visit, for example an extraction and treatment of other teeth, if the plan includes them.

Questions and answers
What does tooth extraction cost?
The clinic’s price list shows from 3 770 UAH for extraction, depending on the complexity category. The surgeon will tell you the category and the full amount after the examination and X-ray, before the extraction.
Does tooth extraction hurt?
Local anaesthesia numbs the area completely, so during the extraction you may feel pressure at most. If you are very anxious, we can offer extraction under sedation. The socket may ache for a few days afterwards, and the surgeon will recommend a painkiller.
How long does an extraction take?
A simple extraction usually takes 5–20 minutes once the anaesthetic has worked. A surgical one takes longer because it depends on the roots; the X-ray shows how long your tooth is likely to take.
How long does the wound hurt after extraction?
The aching is strongest on day 2–3 and then eases, usually within about a week. If the pain gets worse after a few days, call the surgeon.
How soon can I eat after an extraction?
After about two hours, once the numbness has worn off. For the first few days choose soft warm food and chew on the other side.
What should I rinse my mouth with after an extraction?
Do not rinse at all during the first 24 hours, so as not to wash the clot out of the socket. After that, use only what the surgeon recommends and in the way they advise: usually gentle mouth baths rather than vigorous rinsing.
Can a tooth be removed during a dental abscess?
It depends: the surgeon will assess the inflammation and decide whether to remove the tooth straight away or settle the acute infection first. Do not put off the visit and do not warm your cheek.
When can an implant be placed after extraction?
Sometimes on the same day: this is immediate implant placement, possible when the bone around the socket is healthy and sufficient. Otherwise the implant is placed after healing, usually a few months later; the surgeon will give you the timing from the CT scan.
